저출산과 인구구조 변화에 대응하는 보건의료체계 개선 방안 연구

본 연구는 한국의 저출산과 급속한 인구고령화가 보건의료체계에 미치는 영향을 분석하고 개선 방안을 제시한다. 2023년 합계출산율 0.72명, 65세 이상 인구 18.4% 등의 통계를 바탕으로 인구구조 변화의 심각성을 파악하였다. 일본, 중국, 독일 등 국제 사례 비교를 통해 한국의 초저출산·초고령화 동시진행 특성을 도출하였다. 의료수요의 양극화, 의료인력 불균형, 재정 지속가능성 위기 등 보건의료체계의 주요 문제를 분석하였으며, 생애주기별 맞춤형 서비스 강화, 의료인력 재구조화, 재정 지속가능성 확보, 지역사회 중심 통합 거버넌스 구축 등 4가지 개선 방안을 제시한다. 이러한 전략은 예방 중심, 지역사회 기반, 지속가능한 보건의료체계로의 전환을 통해 인구감소 시대의 의료효율성 유지와 국민건강 수준 향상을 가능하게 할 것으로 기대된다.

This study analyzes the impact of Korea's low birth rate and rapid population aging on the healthcare system and proposes improvement measures. Based on statistics such as the 2023 total fertility rate of 0.72 and the elderly population (65+) at 18.4%, the severity of demographic structural changes was identified. Through comparative analysis of international cases including Japan, China, and Germany, Korea's characteristics of simultaneous ultra-low fertility and ultra-aging were derived. Key issues in the healthcare system including polarization of medical demand, healthcare workforce imbalance, and financial sustainability crisis were analyzed. Four improvement strategies are proposed: strengthening customized medical and care services by life cycle, restructuring healthcare workforce, securing financial sustainability, and establishing community-based integrated governance. These strategies are expected to enable maintenance of medical efficiency and improvement of national health levels in an era of population decline through transition to a prevention-focused, community-based, and sustainable healthcare system.
