두부 및 안면 외상 환자의 응급 간호 중재: 병태생리, 임상 평가, 그리고 간호 관리

두부 및 안면 외상은 즉각적인 생명 위협과 장기적 신경학적 손상을 초래하는 고위험 손상으로, 초기 간호 사정과 처치가 환자의 예후를 결정하는 핵심 요인이다. 본 논문은 외상성 뇌손상의 병태생리, 국소성 및 미만성 손상의 분류, 이차 손상 예방 기전을 체계적으로 분석하고, ATLS 및 KTAT 기반의 ABCDEs 응급 평가 프로토콜을 제시한다. 두개내압 관리, 고삼투압 수액요법, 뇌실외 배액 간호, 영상진단 및 수술적 치료 방법을 상세히 기술하며, 안면 외상의 Le Fort 골절 분류, 안와 손상, 눈 외상에 대한 평가 및 치료 원칙을 제시한다. 또한 외상 후 간질, 수두증, 안면신경 마비 등 만성 합병증과 Red Flags 증상에 대한 간호 중재를 강조한다. 본 연구는 다학제적 접근 속에서 간호사의 핵심 역할을 규명하고, 임상 현장에서 즉시 적용 가능한 근거기반 간호 중재 전략을 제공함으로써 두부 및 안면 외상 환자의 생존율 향상과 기능 회복을 도모하는 데 기여한다.

Head and facial trauma represent high-risk injuries with immediate life-threatening complications and long-term neurological sequelae, where initial nursing assessment and intervention are critical determinants of patient outcomes. This paper systematically analyzes the pathophysiology of traumatic brain injury, classifies focal and diffuse injuries, and examines secondary injury prevention mechanisms. It presents the ATLS and KTAT-based ABCDEs emergency assessment protocol, and details intracranial pressure management, hyperosmolar fluid therapy, external ventricular drainage nursing care, neuroimaging, and surgical treatment modalities. Additionally, the paper addresses Le Fort fracture classification for facial trauma, orbital injuries, and ocular trauma assessment and treatment principles. Chronic complications including post-traumatic epilepsy, hydrocephalus, and facial nerve palsy, along with critical Red Flags requiring immediate intervention, are emphasized. This study identifies the pivotal role of nurses within a multidisciplinary approach and provides evidence-based nursing intervention strategies applicable in clinical settings, thereby contributing to improved survival rates and functional recovery in patients with head and facial trauma.
